Yes, Ark Valley Dental accepts Colorado Medicaid (Health First Colorado) as well as CHP+

Yes and No.

Ark Valley Dental is a "fee for service" practice. Payment is due at the time services are rendered.  

Regardless of insurance carrier you may receive dental care by Ark Valley Dental, pllc as an “out-of network” benefit. You may be reimbursed by your dental insurance carrier at a lower reimbursement rate than if you were to receive care by an “in-network” provider. We are happy to submit a pre-treatment estimate to determine coverage prior to your appointment. 

Ark Valley Dental, pllc (Susan Davis, DDS) is not an “in-network” provider with any private dental insurance company. This includes but is not limited to Delta Dental, United, Cigna, Aetna, Principal, Guardian, and many others. 

It is important to understand that the insurance contract is between the insurance company and you, the insured. Our office will gladly submit your insurance claim to your insurance carrier, as a courtesy to you. You are fully responsible for all fees charged by this office regardless of your insurance coverage. 

Dental insurance was not designed to pay for all dental care. Our goal is to maximize the amount of your care covered by your insurance benefits. Treatment recommendations made by our doctor are based on an individual’s needs and not necessarily based on what insurance coverage is available.

There are many steps in restoring teeth with implants. Dr. Davis completes restoration of implants in house (the crown or dentures), but will refer the surgical placement of the implant to a specialist to ensure the highest standard of care is maintained.
